Allogeneic hematopoietic stem cell transplantation (HCT) and advanced cellular therapies, including chimeric antigen receptor T-cell (CAR-T) therapy, are potentially curative treatments for a wide range of malignant and non-malignant hematologic disorders in both pediatric and adult patients. Despite significant therapeutic advances, these procedures remain associated with substantial morbidity and mortality due to complications such as acute and chronic graft-versus-host disease (GvHD), severe infections, relapse, delayed immune reconstitution, and therapy-related toxicities. Increasing evidence indicates that the gut microbiota (GM) plays a critical role in modulating immune responses and influencing clinical outcomes after HCT. Reduced microbial diversity and domination by pathogenic taxa have been associated with higher rates of GvHD, bloodstream infections, transplant-related mortality, and inferior overall survival in adult populations. Conversely, the presence of beneficial commensal bacteria and microbial metabolites, such as short-chain fatty acids, appears to support epithelial barrier integrity and immune regulation. Emerging data also suggest that microbiota composition may influence the efficacy and toxicity profile of CAR-T therapy.
